Community living
The community living program provides support to individuals who live independently, or in a group type accommodation in the Inner West and South West regions (excluding Wincarribee).
A maximum of 15 hours weekly per individual is provided.
Support provided
On an annual basis each client is assessed to determine their needs and support hours. Based on the assessment and meeting with the client an Individual Service Contract is established. This contract outlines what hours will be provided, what support will be
provided and a timeframe for review (formally on 12 months basis).
